  • Overview

    Reliant Medical Group is dedicated to advancing an integrated approach to behavioral health services to provide our patients with access to the care they need.  The integrated behavioral health provider will assess, diagnose and treat patients with various behavioral health and psychological conditions and levels of acuity, as part of an integrated, multi-disciplinary primary care team.

    Responsibilities

    • Assess, diagnose and treat pediatric primary care patients for behavioral health and psychological disorders, at assigned clinical site(s)
    • Provide direct clinical services to patients and families in the form of psychosocial assessments, individual, family, and group treatment.  Collaborates as a member of multi-disciplinary integrated team for the development and delivery of treatment plans 
    • Provide evidence-based treatment interventions in episodes of care that generally last 1 week to four months, with a range of 1-10 visits per episode of care.  Once patient is stabilized, patient will maintain ongoing care with their PCP, knowing they can return to BH provider for another assessment/episode of care as needed
    • Collaborate with and support primary care providers on the ongoing care of patients with mild to moderate behavioral health conditions who can safely and effectively be managed by the PCP/AP; includes patient/parent/guardian follow-up (by telephone or in person) as needed
    • Collaborate and communicate with primary care providers for the care of patients with moderate to complex behavioral health conditions.  Accept warm-handoffs to transition the patient into BH episode of care until stabilized 
    • Provide crisis intervention and stabilization for patients presenting or calling to their primary care office at risk or in need of immediate care
    • Address gaps in access to community BH services by providing interim, back-up or transitional care and patient follow-up (by telephone or in person) with suicidal, high need or high risk patients to monitor and manage risk and safety until they are able to connect with appropriate treatment resource/ provider
    • Provide interventions with patients regarding health behavior change and adherence to medical plans
    • Provide behavioral interventions for patients with various medical conditions
    • Enhance patient engagement in BH care by providing brief problem-focused interventions (e.g. BA- behavioral activation; MI - motivational interviewing, etc.); facilitate treatment including providing episode of care, linking patient back to PCP, or referring to community partner for treatment as needed
    • Present cases to psychiatric prescribers for recommendations to primary care providers for prescribing
    • Perform case management and coordinate care in partnership with medical social worker, and other team members
    • Work closely with case management to ensure follow up with patients (by telephone or in person) to determine what variables (e.g. social determinants of health, etc.) may be affecting patient's ability to attend appointments and/or comply with treatment
    • Facilitate appropriate patient referrals for chronic substance abuse, chronic mental health services, complex care management and other appropriate community resources
    • Work within and contribute to an integrated approach to Behavioral Health care and communication, working collaboratively and professionally with the primary care team, Department of Pediatrics leadership and other Reliant behavioral health colleagues, care management team, and psychiatric prescriber team
    • Obtain and coordinate communication with collateral contacts including school personnel, community based behavioral health providers, and/or state agency caseworkers, to coordinate behavioral health care in collaboration with the PCP
    • Help develop and complete relapse prevention and/or self-management plans with patients as needed. Collaborate in the identification of “at risk/high risk” patients and the development of plans to prevent further psychological or physical deterioration; assist in preventing morbidity in chronic co-occurring conditions that tend to recur over time 
    • Actively participates in PCP and staff education presentations, care coordination meetings, case reviews, utilization management rounds and other department meetings as required or directed.  Attend and participate in Quality Improvement activities and/or research activities as required 
    • Enhances professional growth and development through educational programs, seminars, etc.  Keeps abreast of current clinical literature related to clinical social work and evidence based practices for treatment of pediatric population
    • Maintains patient, family, staff and institutional confidentiality
    • Complies with health and safety requirements and with regulatory agencies such as DPH, etc.
    • Complies with established departmental policies, procedures, and objectives
    • Regular, reliable and predictable attendance is required

    Qualifications

    • Minimum 3 years of experience in a behavioral health environment, working with children and families  
    • Master’s Degree from an accredited school and active
    • Unrestricted licensure as a LICSW OR Doctoral degree in Psychology from an accredited school
    • Licensed as a psychologist to practice independently in the Commonwealth of Massachusetts
    • CANS Assessor certification preferred
    • CPR Certification
    • Current Massachusetts driver’s license
    • Excellent communication, interpersonal and organizational skills, with a flexible and creative approach to problem solving
    • Proficient in computer and EMR use, the Internet and health information technology
    • Successfully able to work independently as well as a member of an interdisciplinary team
    • Possesses good clinical judgment and decision making skills
